Hit the adaptor with a heat gun and when you get it off try using a strip of 400 wet and dry around the shifter shaft to take a thou or so off the shaft dia to see if it will slide on. You do have the correct adaptor don't you. I think it's the 081201.

Looks like the clutch has been overheated for whatever reason which has caused the pressure plate to lose it's flat face and take on a slightly concave profile which has caused the friction plate wear to be greater on the OD.
