There doesn't appear to be any clips holding it on, just seems to slide on and "snap" in from what I can see. It's likely the knobs either aren't being pushed down enough and installed correctly or the inside of the knob where it's supposed to clip in is defective. Either way seems like bad QC on Toyota's part.

Mine happened 8-9 times, but after one good whack in, it has not happened again, granted I am a little more cautious with how I shift to avoid it so that may be a reason why, the first time was pretty scary, mid 1-2 pull and it came off and car threw itself in N and just banged the rev limiter in the middle of an intersection and that metal rod gets super hot so it was a bit hectic getting back into gear and pulled over.

This is common practice in the BMW world.

It's not shocking to see that not many BMW owners went for a Toyota though.

I will say, BMW gearboxes don't like having a hand resting on the shifter or being overly aggressive when shifting.

That's Mustang stuff.

Nothing‘s broken. Manual knobs in BMWs do not thread in. They are popped into place. Whatever reason yours was not pushed or clicked in all the way down. Which led to it popping out very easily. Once you give it a good shove, it will click back in. And you’re good to go. Not a big deal.
